Margarita Palestino says Sheen grabbed her inappropriately and punched her in the chest while making threats during a Sept. 25 appointment.

According to court documents, the actor arrived at the Los Angeles office accompanied by a body guard and personal assistant and started “yelling incoherently” at Palestino and the dental staff.

While being attended to by the technician and dentists George Bogen and Jaime Azdair, Sheen allegedly ripped off a nitrous oxide mask placed on his face and jumped out of the chair, causing dental instruments to fall on the floor.

He then yelled that he was going to kill the staff, after which Bogen attempted to calm him down, the lawsuit said. When Palestino leaned over the place the mask on Sheen’s face a second time, he allegedly grabbed her breast and pulled her scrubs, trying to open or remove her bra, she said.

Azdair added that Sheen lunged at him with a knife but could not reach him and instead stabbed the chair in the examination room.

Spit covered the walls of the room and items that “were broken or in disarray” lay scattered throughout the area, according to court documents.

Before Sheen left the dental office, his personal assistant approached Palestino and handed her $500 “for your troubles,” she said.

Bogen alleged the actor was “high” on a combination of crack cocaine, Theradol, and alcohol.

“Our client is the victim of heinous acts perpetuated by Charlie Sheen,” said Neama Rahmani of West Coast Trial Lawyers, the law firm representing Palestino. “She will not be intimidated by Sheen or those who are attempting to cover up for him. We look forward to exposing the truth and vindicating her rights in court.”
